Can I send representative if I want to proceed with termination at TMpoint?

 

If you could not come to TMpoint due to below condition you may send representative for termination request:

 

Residential and Business (registered as personal/trading/enterprise)

Category

Supporting document required

Customer have health problem /unable to walk-in to TMpoint/ Critical Illness

 

• Doctor’s letter on account owner health condition/ Medical appointment card/any medical document.

• A copy of owner’s IC/Passport

• Authorization person’s IC/Passport

• Authorization letter with owner’s sign off

 

Customer are not in Malaysia (resides / working overseas)

 

• A copy of passport or letter study oversea/work overseas

• A Copy of owner’s IC/Passport

• Authorization person’s IC/Passport

• Authorization letter with owner’s sign off.

 

OKU

 

• A copy of owner’s IC/Passport

• Authorization person’s IC/Passport

• Authorization letter with owner’s sign off.

• Owner’s OKU card

 

Senior Citizen ( 60 years and above)

 

• A copy of owner’s IC/Passport

• Authorization person’s IC/Passport

• Authorization letter with owner’s sign off.

 

Account owner has died

 

• Authorization person’s IC/Passport

• Original / Certified true copy of Death certificate of old account owner

 

 

Business (registered as SDN BHD /BHD/LLP/ Ikhtisas Professional (Registered)/Company without Business Registration Number (BRN)


A company representative may apply for the termination on behalf of the
company with the supporting documents below:
• A copy of the Director’s IC/Passport
• The representative’s IC/Passport
• Original Authorisation letter with the company letterhead
• Business Registration related form
