As far as I'm concerned, unifi did not generate/keep cust's yearly statement. However, you may retrieve your monthly billing statement for year 2021 via:
(1) myunifi app or unifi selfcare portal (unifi.com.my) for latest 6months billing statement.
You might be able to retrieve your October - December 2021 if you login and download the billing statement before entering new month (April 2022). For January - September 2021, you can try option (2)
(2) Check inbox/junk/spam of the registered email in your billing profile.
You may download the PDF bill sent to the registered billing email.

Income tax dont require you to provide the whole year bill. The last 6 months will do fine as it will average your usage. Also, the plans will show the amount for monthly.
Unless u gone oversea and have international call, check your email. all bills will be there
